Applicant must have a cut-off notice from the utility company. A copy of the cut-off notice must be provided at the appointment. To make an appointment, call 918-582-1356 on Monday or Tuesday between 9:00AM and 10:30AM*. A photo ID and social security card must be presented at the appointment.
